But when a patient opens ChatGPT and asks "What are the best dermatology practices in Dallas for Botox?" that group does not appear. When a family member asks Perplexity "Which dental offices near me offer same-day implants?" the practice that ranked first on Google is not in the answer. When someone searches Google AI Mode for "PHP programs in Phoenix that take Aetna," the treatment center that built its SEO for years comes up invisible.
This is the entity authority problem, and it is the most common reason healthcare practices lose AI search visibility despite having strong traditional rankings.
77% of Google page-one businesses are invisible in ChatGPT. The gap exists because traditional search and AI search use fundamentally different visibility mechanisms. What entity authority actually means
An entity, in the context of AI and search engines, is any distinct, real-world thing that can be identified and described. Your practice is an entity. Your medical director is an entity. "Intensive Outpatient Program" is an entity. "LASIK surgery" is an entity. "Invisalign" is an entity.
Entity-based SEO represents the shift from optimizing for search terms to becoming a recognized concept within search engines' knowledge graphs. For brands competing in AI-powered search, entity recognition determines whether you are cited as an authority or overlooked entirely.
Entity authority is the degree to which AI engines can confidently answer three questions about your practice:
Who are you? Is your practice name, location, specialty, and contact information consistent and verifiable across multiple independent sources? Can an AI engine cross-reference what your website says against what SAMHSA, the AAD, the ADA, Healthgrades, and Google Business Profile say, and find the same information?
What do you do? Are the services you offer described consistently and specifically enough that an AI engine can map them to the clinical categories it recognizes? Does your site use the same terminology for your services as the governing bodies and directories in your specialty?
Why should you be trusted? Are you cited, listed, accredited, and mentioned by sources that AI engines already recognize as credible? Does your entity connect to a broader ecosystem of verified information, or does it exist in isolation on your own website?
AI-driven systems now operate on structured ecosystems of entities. The goal is no longer to rank for a term. It is to become the verified authority within an interconnected system of entities. The search engine has become a reasoning engine. It looks at your content and the logical role your brand plays within a broader ecosystem.
A practice that cannot be verified across independent sources, that uses inconsistent naming across directories, or that exists only on its own website is an unverified entity. Unverified entities do not get cited.
Why healthcare practices are especially vulnerable
Healthcare practices face a specific version of the entity authority problem that most general guidance does not address.
The clinical vocabulary problem is real across every specialty. A treatment center might call it "Partial Hospitalization Program" on their website and "PHP" in their SAMHSA listing. A dental office might say "clear aligners" on their service page and "Invisalign" in their Google Business description. A dermatology group might list "laser skin resurfacing" on their site but "fractional CO2 laser" in their RealSelf profile. A physical therapy clinic might describe "manual therapy" without connecting it to the specific technique categories their referring physicians use.
Each of these inconsistencies is an entity mismatch.
AI systems do not just read your location page in isolation. They cross-reference it against everything else they can find about your business. If your NAP is inconsistent across directories, your hours are outdated on Apple Maps, or your service categories do not match between Google Business Profile and your website, the AI's confidence in your entity drops. Lower confidence means lower citation probability.
The multi-location problem amplifies this for practices with more than one site. Each location needs to be a distinct, consistently described entity with its own verified presence. A six-location physical therapy group that manages all locations under one Google Business Profile, or that uses a single SAMHSA listing for multiple treatment center campuses, is creating entity ambiguity that AI engines cannot resolve.
The provider-level problem means the entity issue extends beyond the practice to the individual clinicians. In healthcare, the clinician is often the entity that patients are actually searching for, especially in mental health, dermatology, and plastic surgery. A practice whose clinical staff have no individual entity presence in specialty directories, no linked professional profiles, and no bylined content is missing the provider-level signals that AI engines weight heavily in healthcare queries.
Every inconsistency in name, address, specialty listing, or insurance information is a signal to AI models that the entity data is unreliable. Unreliable entities do not get cited. The practices that build AI search authority in 2026 will be the ones that new patients find first in 2027, 2028, and beyond.
The three layers of entity authority for healthcare
Building entity authority is not a single task. It operates at three layers simultaneously, and gaps at any layer suppress citation probability regardless of how strong the other two layers are.
Layer 1: The practice entity
This is the foundational layer. It covers how your practice as an organization is understood and verified by AI engines.
NAP consistency across all surfaces. Your practice name, address, phone number, and website URL must be identical across every place they appear: your website, Google Business Profile, Apple Maps, Yelp, Healthgrades, Zocdoc, specialty association directories, and any local business directories. Not similar. Identical. Abbreviations, suite number formats, and phone number formatting all matter. A listing that says "Ste 200" and another that says "Suite 200" is a mismatch that reduces entity confidence.
Business category accuracy. Your Google Business Profile primary category and secondary categories must accurately reflect your specialty. A dental practice listed primarily as a "Health and Beauty Shop" because someone chose the wrong category years ago is an entity mismatch. A treatment center listed as a "Rehabilitation Center" rather than a "Drug Addiction Treatment Center" may miss the category signals AI engines use to route behavioral health queries.
Service description consistency. The services you list on your website should use the same terminology as your specialty association directories. For treatment centers, align with SAMHSA's service category labels. For dental, align with ADA procedure codes and common patient-facing terminology. For dermatology, align with AAD service categories. For physical therapy, align with APTA clinical practice categories. This is how AI engines map your services to the queries they are answering.
Schema at the organization level. Your website needs a MedicalOrganization or LocalBusiness schema block that defines your entity in machine-readable terms: your legal name, your doing-business-as name if different, your address, your phone, your specialty, your geographic service area, your accreditations, and your sameAs links. The sameAs property is critical: it tells AI engines that your website entity is the same entity as your Google Business Profile, your Healthgrades listing, your SAMHSA entry, and your specialty association directory listing.
sameAs links connect your entity to authoritative external references. This process, known as entity disambiguation, signals to AI exactly who you are in the global knowledge ecosystem.
Layer 2: The service and location entities
Each service you offer and each location you operate is a distinct entity that needs its own consistent, verifiable presence.
Service-level entity definition. Every major service line needs a dedicated page that defines the service as an entity: what it is, who it is for, what the clinical process involves, and how it relates to other services you offer. For a treatment center, that means distinct pages for detox, residential, PHP, IOP, and outpatient that describe each level of care specifically. For a dental practice, that means distinct pages for implants, orthodontics, cosmetic, and restorative that describe each service in patient-facing clinical terms. Generic "services" pages that list everything in bullet points are not entity definitions. They are indexes.
Location-level entity definition. Each physical location needs its own Google Business Profile, its own page on your website, and its own presence in location-specific directories. The location page on your website should use LocalBusiness schema with the specific address, phone, hours, and service area for that location. It should link to the Google Business Profile for that location via sameAs. It should reference any location-specific staff or clinical directors by name.
Internal linking that maps relationships. AI engines understand your entity ecosystem partly through how your pages link to each other. A service page that links to the clinical staff who deliver that service, to the conditions it treats, and to the location where it is available is helping AI engines build a connected understanding of your entity relationships. A service page that exists as an island, linked from the nav menu but not from any other content, contributes much less to entity clarity.
Layer 3: The people entities
In healthcare, the clinicians are often the most trusted entities in any patient decision. A practice whose clinical staff have no individual entity presence is missing the layer that AI engines weight most heavily for YMYL health queries.
Provider directory listings by specialty. Each clinician at your practice should have a claimed and complete profile in the primary directory for their specialty. For behavioral health: Psychology Today, SAMHSA, ASAM member directory. For dental: ADA member directory, Zocdoc, Healthgrades. For dermatology: AAD member directory, RealSelf, Healthgrades. For plastic surgery: ASPS member directory, RealSelf, Healthgrades. For ophthalmology: AAO member directory, Healthgrades, Zocdoc. For physical therapy: APTA member directory, WebPT, Healthgrades. For med spas: American Med Spa Association (AmSpa) directory, RealSelf.
Bylined content on your own site. Every clinical page should be authored or reviewed by a named clinician whose credentials are visible. As covered in our post on E-E-A-T for healthcare practices, the clinician bio page is the entity definition for that individual: their name, credentials, specialty, license number, institutional affiliations, and a photo. The bio page becomes the sameAs reference point that connects the person named on your clinical content to the verified provider entity in specialty directories.
Earned mentions in clinical contexts. A clinician quoted by name in a trade publication, cited in a clinical case study, or listed as a presenter at a specialty conference is building individual entity authority that flows back to your practice. Provider-level earned mentions are among the highest-value entity signals in healthcare.
The specialty-specific entity sources that matter most
Generic business directories contribute little to healthcare entity authority. The sources that actually move AI citation probability are specialty-specific, and the list is different for each vertical.
Treatment centers and mental health clinics
Primary entity sources: SAMHSA Treatment Locator (findtreatment.gov), Psychology Today therapist directory, NAATP member directory, ASAM member directory, CARF or Joint Commission accreditation registry, state licensing board listing, LegitScript verified advertiser registry.
Entity consistency priority: SAMHSA listing name and address must match your website and Google Business Profile exactly. This is the single most cited entity source for addiction treatment queries and the most common source of entity mismatch on treatment center sites.
Dental practices
Primary entity sources: ADA member directory (ada.org/find-a-dentist), Zocdoc, Healthgrades, 1-800-Dentist, Google Business Profile (category: Dentist or relevant specialty), state dental board license verification, AGD fellowship listing if applicable.
Entity consistency priority: Provider NPI numbers should be consistent across all listings and match the CMS NPI Registry. ADA specialty board certifications (endodontics, periodontics, oral surgery, orthodontics) should be listed on both the provider bio page and the relevant specialty directory profiles.
Dermatology practices
Primary entity sources: AAD member directory (aad.org/public/find-a-derm), ASDS member directory, RealSelf practitioner profile, Healthgrades, Zocdoc, Google Business Profile (category: Dermatologist), state medical board license verification.
Entity consistency priority: Procedure names on your website should align with AAD and ASDS standard terminology. A practice that calls a treatment "laser skin tightening" on their site but "radiofrequency microneedling" on their RealSelf profile is creating a service entity mismatch. Pick the most recognized clinical term and use it consistently everywhere.
Plastic surgery and med spas
Primary entity sources: ASPS member verification (plasticsurgery.org/find-a-surgeon), ASAPS member directory, AmSpa member directory (for med spas), RealSelf practitioner profile, Healthgrades, Google Business Profile (category: Plastic Surgeon or Medical Spa), state medical board license verification, ABPS board certification verification.
Entity consistency priority: Board certification status must be stated the same way across every surface. "Board Certified Plastic Surgeon" on your website needs to match your ASPS profile verification status exactly. Unverified or inconsistent board certification claims are entity red flags.
Ophthalmology practices
Primary entity sources: AAO member directory (aao.org/find-an-ophthalmologist), ASCRS member directory, ABMS board certification verification, Healthgrades, Zocdoc, Google Business Profile (category: Ophthalmologist), state medical board license verification.
Entity consistency priority: Subspecialty terminology needs to be consistent. A practice that offers retina care should use "retina specialist" or "vitreoretinal surgeon" consistently across their website, AAO listing, and Healthgrades profile, not switching between terms across platforms.
Physical therapy clinics
Primary entity sources: APTA member directory (apta.org/find-a-pt), ABPTS specialty certification registry, WebPT practitioner listing, Healthgrades, Zocdoc, Google Business Profile (category: Physical Therapist), state physical therapy board license verification.
Entity consistency priority: Clinical specializations (orthopedic, neurological, sports, pediatric, pelvic health) should be labeled consistently across your website, APTA listing, and Healthgrades profile. A therapist who specializes in pelvic floor PT should have that specialty listed explicitly and consistently, not described with varying terms across different platforms.
The entity authority audit: where to start
Before building new entity signals, audit what exists. The most common finding: inconsistencies that are actively suppressing citation probability that a few hours of cleanup work can resolve.
Run your own name through every primary source. Search your practice name in Google, ChatGPT, and Perplexity. Note what appears, what is missing, and what is inaccurate. This is your baseline entity snapshot.
Audit NAP consistency across your top five sources. Pull your listing from Google Business Profile, Healthgrades, your specialty association directory, your website, and one additional source specific to your vertical. Compare name, address, phone, and website URL character by character. Fix every mismatch before building new citations.
Check your schema implementation. Use Google’s Rich Results Test (search.google.com/test/rich-results) on your homepage and your primary service pages. Verify that your organization schema is present, that it includes sameAs links to your major directory listings, and that it correctly identifies your specialty and service area.
Inventory your provider directory completeness. For each clinician at your practice, check whether they have claimed and complete profiles in the two or three primary directories for your specialty. Note which are missing, which are unclaimed, and which have outdated information.
Check for entity isolation. Identify your five most important service pages. Do they link to the clinicians who deliver those services? Do they link to the locations where those services are available? Do they link to related services? Pages that exist without these connections are entity islands that contribute less to your overall entity authority than they should.
Before expanding your citation footprint, always audit and clean up what exists. Many local businesses discover that fixing existing citation errors improves their position faster than adding dozens of new listings.
Schema: the machine-readable layer of entity authority
Every entity signal described above should be reflected in your schema markup. Schema is not optional for healthcare practices competing in AI search.

The schema types that matter most for healthcare entity authority:
Organization or MedicalOrganization schema
Defines your practice as an entity. Required fields: name, url, logo, address, telephone, specialty, sameAs (linking to Google Business Profile, Healthgrades, your specialty association directory, and SAMHSA if applicable).
LocalBusiness schema
Applied to each physical location. Required fields: name, address, telephone, openingHours, geo (latitude and longitude), sameAs (linking to the Google Business Profile for that specific location).
MedicalTherapy or MedicalProcedure schema
Applied to service pages. Defines each service as a recognized clinical entity. Required fields: name, description, recognizingAuthority (the governing body that defines this service category), relevantSpecialty.
Person schema
Applied to clinical staff bio pages. Required fields: name, jobTitle, worksFor, hasCredential (license type and number), sameAs (linking to their Healthgrades profile, specialty directory listing, and any other verified provider profile).
FAQPage schema
Applied to pages with FAQ sections. This is the highest-leverage schema type for AI citation because AI engines pull FAQ content at a disproportionately high rate. Every service page and condition page should have a substantive FAQ section with FAQPage schema applied.
sameAs creates a consistent identifier that connects related entities across your website and across the web, ensuring AI understands they belong to the same source and are the same real-world entity. Without sameAs links connecting your schema entities to your directory listings, your schema describes entities that exist in isolation rather than as part of a verified, interconnected presence.
How entity authority compounds over time
Entity authority is not a campaign. It is a compound asset.
Every new directory listing that matches your existing NAP data strengthens the entity confidence signal. Every trade press mention that names your practice or your clinicians adds a new node to your entity graph. Every FAQ answer on a well-structured page that gets cited in an AI response trains the AI engine to recognize your practice as a reliable source for that category of query. Every clinician bio that earns a mention in a clinical context adds to the provider-level entity signal.
The goal is to make your brand easier for AI systems to trust, cite, and recommend, which matters more as zero-click search grows and AI tools shortlist only a few brands.
The practices that will have the strongest AI search visibility in 2027 and 2028 are the ones that started building their entity authority systematically in 2026. The gap between a practice with strong entity authority and one without is a compounding gap: every new piece of content, every new citation, every new directory listing works harder for a practice with a strong entity foundation than for one without it.
